It appears that the problem is with the Engine Coolant Temperature (ECT) sensor or its wiring. When the ECU doesn't get a signal from the ECT, it doesn't know if the coolant is too hot or not and so as a failsafe, it runs the fan at 100% to avoid overheating.The sensor had failed, making the gauge read "0". As a failsafe since it has no idea what the actual coolant temp is, the PCM commands both fans on high speed and keeps the A/C off to prevent extra heat from the condenser.
